The aircraft landed safely on Rickenbacker's runway 23R about 20 minutes later. The aircraft vacated the runway and stopped for an inspection by emergency services, about 20 minutes later the aircraft taxied to the apron. The aircraft remained on the ground for about 19 hours, then positioned back to Louisville and resumed service. https://avherald.com/h?article=54004f48&opt=0 Incident: Garuda B738 at Jakarta on Sep 3rd 2026, blew tyre on departure A Garuda Boeing 737-800, registration PK-GFS performing flight GA604 from Jakarta to Makassar (Indonesia) with 141 passengers and 8 crew, was climbing out of Jakarta when the crew was informed a tyre had blown on departure. The crew continued the flight to Makassar, performed a hold, a low approach to Makassar's runway 03 and landed safely on runway 03 about 2:20 hours. The inboard right hand main tyre was found blown. A replacement Boeing 737-800 performed the next sector. The occurrence aircraft is still on the ground in Makassar about 18 hours after landing. https://avherald.com/h?article=54004877&opt=0 Incident: Aeromexico B38M near Mexico City on Sep 2nd 2026, power bank thermal runaway An Aeromexico Boeing 737-8 MAX, registration XA-DAP performing flight AM-371 from Tapachula to Mexico City (Mexico), was enroute when the power bank of a passenger on an empty seat suffered a thermal runaway. The device was secured. The aircraft landed safely on Mexico City's runway 05R. The power bank was handed to emergency services. The aircraft is still on the ground about 25 hours after landing. https://avherald.com/h?article=540043e7&opt=0 Incident: Shenzhen A333 at Haikou on Sep 1st 2026, temporary runway excursion A Shenzhen Airlines Airbus A330-300, registration B-1036 (dep Aug 31st) performing flight ZH-9313 from Shenzhen to Haikou (China), had gone around from about 150 feet AGL due to weather on first approach to Haikou's runway 27, then entered a hold for about 40 minutes before positioning for another approach to runway 27. The aircraft landed on the runway 27 about 60 minutes after the go around. Scratches were found on the left hand main tyres. Subsequently a runway inspection revealed three damaged runway edge lights. The aircraft is still on the ground in Haikou 4 days later. https://avherald.com/h?article=54003845&opt=0 Incident: India Express A20N near Lucknow on Sep 3rd 2026, cargo smoke indication An Air India Express Airbus A320-200N, registration VT-ATD performing flight IX-1253 from Varanasi to Delhi (India) with 155 people on board, was enroute at FL320 about 50nm northwest of Lucknow (India) when the crew decided to turn around and divert to Lucknow due to a cargo smoke indication. The aircraft landed safely on Lucknow's runway 09 about 25 minutes later. A replacement A320-200 registration VT-EXA reached Delhi with a delay of about 5:15 hours. The occurrence aircraft is still on the ground in Lucknow about 14 hours after landing. https://avherald.com/h?article=54003427&opt=0 Incident: Indigo A21N near Muscat on Sep 3rd 2026, pilot incapacitation An Indigo Airbus A321-200N, registration VT-ILO performing flight 6E-1302 from Doha (Qatar) to Bangalore (India) with 155 people on board, was enroute at FL370 about 100nm southeast of Muscat (Oman) when the aircraft turned around and diverted to Muscat due to one of the pilots becoming incapacitated. The aircraft landed safely on Muscat's runway 26R about 25 minutes later. The aircraft remained on the ground in Muscat for about 9 hours, then continued the journey and reached Bangalore with a delay of about 10 hours. https://avherald.com/h?article=540031b4&opt=0 Air India Phuket Flight Pilot Tested Positive For Psychoactive Substance: Earlier, the pilot tested positive for psychoactive substances during the post-flight screening in Delhi. Another test was conducted sometime later, which also turned out to be positive. The co-pilot of Air India flight A12379 from Phuket to Delhi, which experienced a 300 feet altitude drop during its cruise phase over Odisha, tested positive for psychoactive substance, says a preliminary report investigating the incident. "Detailed investigation regarding technical and other aspects are underway. In this case, it has been observed that the PIC was found non-negative for psycho-active substance in the confirmatory test. The confirmation of use of psycho-active substances is a serious concern and AAIB recommends appropriate actions by DGCA on priority," said the preliminary report by the Aircraft Accident Investigation Bureau (AAIB). Earlier, the pilot tested positive for psychoactive substances during the post-flight screening in Delhi. Another test was conducted sometime later, which also turned out to be positive. The test reportedly confirmed that the pilot had smoked marijuana, which is commonly known as weed. "As the flight originated from a destination outside India, therefore, on arrival at Delhi airport, both flight crew were subjected to post flight medical (Breath-Analyser examination). The Breath Analyser examination report was found satisfactory," the report said. "In addition, both flight crew were subjected to Psycho-active substance detection test by using Psycho-active substance detection kit. The sample of PIC was found non-negative. Subsequently, blood samples of both the flight crew were sent to the pathological laboratory for confirmatory test. The confirmatory test report of PIC was found non-negative whereas the sample of the Co-Pilot was found negative in both the tests," it said. There were 145 persons on board: 137 passengers, including 3 infants, 6 cabin crew and 2 flight crew members. At least 20 passengers and four crew members suffered injuries. It was initially notified that the aircraft encountered turbulence enroute to Delhi, but landed safely. An AAIB team arrived at the airport to assess the situation. Based on the evidence collected and the assessment of injuries sustained by the persons on board, the occurrence was later classified as an accident. The Airbus A320 aircraft has three hydraulic systems. They are identified as the green, blue and yellow systems. Together they supply hydraulic power at 3000 psi (206 bar) to the main power users. The aircraft first detected loss of green hydraulic system, the report says, adding a little later, it detected the loss of Blue and Yellow hydraulic system. "The Co-Pilot, who was the Pilot Flying (PF) responded and tried to control the prevailing situation. Subsequently, PIC took over the controls. Meanwhile, the aircraft encountered altitude upset. Initially, it gained 372 ft and fell by 292 ft from its assigned FL360 during the event," the report said. A short while later, the Blue hydraulic system was recovered, and few seconds later, the Yellow hydraulic and the Green hydraulic systems were also recovered, the report said. "Consequently, this led to the recovery of the Flight control surfaces. Subsequently, the aircraft operation returned to its normal operating condition," it said. The aircraft sustained damages at several places inside the cabin, the report said, mainly to the hatrack (overhead stowage bins) and ceiling panels. "Prior to departure from Phuket International Airport, pre-flight inspection was carried out by the maintenance personnel and no discrepancies were recorded except for the already invoked Minimum Equipment List (MEL). As per flight crew statement, they did the walk around inspection of the aircraft and nil discrepancies were found," the report said. An investigation into an Air India flight that dropped almost 300 feet last month has found that the plane suffered a temporary hydraulics failure. Flight AI2379 was travelling from Phuket, Thailand to the Indian capital of New Delhi on 4 August when the incident occurred. The Airbus A320, which had 137 passengers, six cabin crew and two pilots on board at the time, suddenly lost altitude while flying over Indian airspace, leaving 24 people injured. In a preliminary report published on Friday, India's Aircraft Accident Investigation Bureau (AAIB) said the aircraft detected a triple-hydraulic failure, with the autopilot disengaging and a stall warning triggered for two seconds. "The aircraft encountered altitude upset. Initially, it gained 372 ft and fell by 292 ft," the report said. The hydraulic systems then recovered, and the "aircraft operation returned to its normal operating condition." The report added that both pilots were tested for psycho-active substances following the incident. "The sample of PIC (pilot in command) was found non-negative. Subsequently, blood samples of both the flight crew were sent to the pathological laboratory for confirmatory test. Air India has since reportedly ordered one-time drug tests for its pilots. FAA Proposes Ending Special-Issuance Requirement For Some Pilots With Diabetes [Credit: Volha_R | Shutterstock] The FAA on Thursday proposed changing its medical certification rules to allow pilots whose diabetes is controlled using non-insulin medications to potentially receive first-, second- or third-class medical certificates without going through the agency’s Special Issuance process. The proposed rule is scheduled for publication in the Federal Register on Friday. AMEs Could Issue Certificates Under current regulations, diabetes that requires insulin or another hypoglycemic drug for control is considered a specifically disqualifying condition. Pilots using non-insulin medications can still qualify for a medical certificate, although an aviation medical examiner generally must defer the application to the FAA for Special Issuance review. The proposal would remove non-insulin medications from that provision and allow qualified applicants to be evaluated under the FAA’s general medical standards. “The availability of numerous new diabetes medication classes and other non-insulin treatments has substantially improved disease management,” the FAA said in the proposed rule. The agency noted that the diabetes standard dates to 1959 and said advances in treatment have reduced severe complications including cardiovascular events and hypoglycemia. The FAA estimates the change could allow AMEs to issue medical certificates at the time of examination in about 3,000 cases each year. Another 1,200 cases currently handled through Special Issuance could potentially transition to AME-managed certification. Over five years, the agency estimates the change would save applicants between $39.7 million and $80.2 million in labor costs. “Processing all NIDDM cases via the SI process places a burden on applicants and FAA,” the agency said. The FAA expects the proposal to reduce certification wait times and its own processing workload while shortening the process for pilots whose diabetes is well controlled. Safety Review As part of the proposal, the FAA reviewed 51 fatal accidents from 2008 through 2025 involving pilots who were reported to be using non-insulin diabetes medications. The agency determined that diabetes, related complications or diabetes medication were unlikely to have contributed to 50 of those accidents. One case remains under investigation. The proposal would not change the requirements for pilots whose diabetes is treated with insulin, who would continue to require Special Issuance authorization. Pilots using non-insulin treatments could also be denied or deferred on a case-by-case basis when the FAA or an AME determines the condition or treatment could interfere with the safe exercise of pilot privileges. This message from the specialized agency of the United Nations comes as the risk of weapons targeting civilian planes grows worldwide. Long-range weapons systems, unmanned aircraft systems (UAS), global navigation satellite system radio frequency interference, and advanced air defence systems are among the military technologies jeopardizing civilian aviation. ICAO is observing heightened risks of civil aircraft being targeted or caught in crossfire, and while the aviation industry is able to reroute flights and maintain operations during conflicts, this operational flexibility does not address the fundamental security threat. Article 3 bis of the 1944 Convention on Civil Aviation and ICAO Assembly Resolution A42-4 explicitly prohibit the use of weapons against civilian aircraft. As reaffirmed in last year’s unanimous adoption of ICAO Assembly Resolution A42-4: Addressing risks to civil aviation arising from conflict zones, it is essential that Member States assess the safety of airspace controlled by them and take all appropriate measures. Aircraft operators must also conduct safety and security risk assessments in accordance with applicable ICAO provisions and guidance. Member States must share threat intelligence rapidly when activities pose risks to civilian aircraft, strengthen risk assessment mechanisms and timely decision making, and improve coordination between military and civilian authorities to prevent misidentification of civilian aircraft as military targets. ICAO is updating its Manual Concerning Safety Measures relating to Military Activities and its Risk Assessment Manual for Civil Aircraft Operations Over or Near Conflict Zones, which focus on how authorities and airlines and air navigation services providers can better assess threats from malicious activities. Powerplant failures and collisions during takeoff and landing were among the leading safety concerns for business aviation during the second quarter of 2026, according to the latest analysis from the National Business Aviation Association (NBAA). NBAA’s review identified 66 reportable accidents and incidents involving turbine-powered business aircraft from April through June. The total included 24 accidents and 42 incidents. Six of the accidents were fatal, resulting in 22 deaths. Powerplant system or component failures were involved in nine of the 66 events, including two of the six fatal accidents. That made powerplant failures the most common occurrence category among the quarter’s fatal accidents. Collisions during takeoff and landing were the most common accident type overall, with five accidents reported. One was fatal and four were nonfatal. The data also showed a significant number of bird strikes, which were involved in 12 events during the quarter. Non-powerplant system or component failures accounted for another 11 events. NBAA said the quarterly analysis is intended to help operators identify emerging safety concerns rather than serve as a determination of probable cause. Tammy Duckworth and Dick Durbin harshly criticized FAA Administrator Bryan Bedford for holding up a pay raise for air traffic controllers that Congress approved as part of legislation President Trump signed into law in April. The Homeland Security and Further Additional Continuing Appropriations Act set aside $140 million to give controllers a 3.8% pay raise, but they have so far only received the 1% raise given to all government employees. In a letter Friday, obtained exclusively by CBS News, the senators call on the Federal Aviation Administration to immediately roll out the rest of the pay hike. "Holding air traffic controllers' remaining 2.8 percent pay increase hostage to force a workforce that has operated under mandatory overtime for years to work longer hours is petty, disrespectful and counterproductive," the senators wrote. Under Bedford, the FAA recently submitted its 2026 Controller Workforce Plan, which the agency says identifies opportunities to improve scheduling efficiency with a goal of raising the average hours of productive time per eight-hour shift from four hours to five. Duckworth is the ranking Democrat on the Senate Aviation subcommittee and Durbin, who is retiring in January, is the No. 2 Senate Democrat. Their letter references a recent interview with the Washington Examiner in which Bedford linked the raise to changes in controller scheduling and workforce utilization to "unlock more productivity." "Squeezing more 'utilization' out of air traffic controllers who have been understaffed, underpaid and overworked for years will weaken retention and undermine aviation safety," the senators wrote in their letter Friday. "Air traffic controllers are operating a strained system, using antiquated technology, while adapting to air traffic control modernization in real time." "Controllers work long, high-stress hours and make split-second decisions that keep millions of passengers safe every day," they continued. "The last thing our National Airspace System needs is a cynical FAA Administrator so hellbent on stretching them thinner that he is willing to withhold their modest 2.8 percent pay raise." Some controllers tell CBS News that the delay in receiving the pay hike is impacting morale and becoming a point of contention. The measure Congress passed included statutory conditions related to the controller pay raise calling for "improvements in workforce scheduling, staffing utilization, or other operational efficiencies ... that contribute to addressing workforce shortfalls and enhancing aviation safety" as determined by the FAA administrator "in his sole discretion." The senators say those conditions are "not a high bar," arguing the FAA's failure to reach an agreement with controllers "means that what you are requesting must not benefit controllers." "As the statute makes clear, Congress—not the FAA—made the pay increase contingent on the FAA Administrator determining that improvements in workforce scheduling, staffing utilization, or other operational efficiencies have been achieved that contribute to addressing workforce shortfalls and enhancing aviation safety," the FAA told CBS News in a statement. "The FAA looks forward to talking with NATCA to review historic scheduling practices and identify opportunities to achieve more productive schedules that will improve safety and controller quality of life." The National Air Traffic Controllers Association, or NATCA, told CBS News, "We continue to advocate for meaningful improvements for controllers, including the pay parity wage increase passed by Congress, through ongoing engagement with the FAA. We appreciate the support of our allies in Congress who understand the importance of recognizing the hard work of our nation's air traffic control workforce." The FAA has been focused on increasing air traffic control hiring as it works to reduce a staffing crunch that has left most FAA facilities understaffed. A 2025 CBS News report found more than 90% of air traffic control towers were understaffed. Last month, Transportation Secretary Sean Duffy announced the FAA had hired a record number of controllers this year after the agency targeted video gamers as potential candidates. "Just in this calendar year, we hired more than 2,000 controllers, got them through the academy and got them hired," Duffy told CBS News in August. "We had over 12,000 applicants in less than 24 hours when we put out our pitch to gamers, and I think it was 8,000 that were qualified. When you get better applications, more applications, we get to pick the best and the brightest to get into the system."
